Definition
Fowler's syndrome is a rare cause of urinary retention in young women: the urethral sphincter does not relax, and the bladder can no longer empty normally.
Causes
The exact cause remains unclear; the syndrome is often associated with polycystic ovary syndrome.
Diagnosis
- Sphincter electromyography, which shows a characteristic activity.
- Urodynamic assessment confirming a full bladder without effective contraction.
Treatments
Intermittent self-catheterisation empties the bladder day to day. Sacral neuromodulation, a small implanted stimulator, restores spontaneous voiding for many patients. Follow-up takes place in specialised urology.
Between catheterisations, small leaks are possible. A thin bladder control pad is often enough; it is changed discreetly and protects the skin.
